MEDICAL CARE SERVICES
At SLM Services, our licensed nurses provide the following in home medical care services
Home health Care - Home health services provided to adults, seniors, and pediatric clients who are recovering after a hospital or facility visit. These services include short-term nursing, rehabilitation and assistive home health care.
Pediatric Care - medical care services provided to patients under the age of 18 to 21 years of age. Our services include, but are not limited to, companion care, wound care, medication administration.
Private Duty Nursing - Provides long term medical care in patients home.
Care Management - Conduct in-home visits to ensure patients care requirements are being followed after facility stay. The services can include: medication reconciliations, diet monitoring, communication with medical team to confirm care regimens.
VACCN HOME CARE SERVICES
VACCN was developed to aid Veterans with access quality healthcare closer to their homes. At SLM Services, we care about our Veterans and are here to make life easier for you and your loved ones. Our wide range of home care services are designed to meet all your health care needs. Our personal care attendants are available to assist you with activities of daily living, companion care, cleaning and transportation. Contact your VA social work for more information on the VACCN program and request SLM Services for your home care services.
NON MEDICAL CARE/ PERSONAL CARE SERVICES
At SLM Services our professional and reliable caretakers have extensive experience with elderly care and disabled care. Our caretakers will provide services like personal care assistance, meal preparation and transportation. Our caretakers are available to service clients on 24 hour shifts, 7 days per week. Our caretakers will assist patients with remaining independent at home. We also provide companion care in nursing home and assisted living facilities.
LONG TERM CARE ASSESSMENTS
SLM Services staffs RN(s) to conduct one time virtual, telephonic or face to face visit to complete required long term care assessments.
